Additionally, students will need to have completed 3 reference forms, provided by CURE.
RECOMMENDATION/REFERENCE FORMS
Two of the three recommendation forms must be completed by professional associates. This can be a guidance counselor, teacher, employer, co-worker, etc. The other recommendation form must be completed by a personal reference and can be completed by anyone the applicant has known for greater than one year. Recommendation letters can be included in the application, however, these must be in addition to the three mandatory reference forms.

Students who have not taken college level Anatomy and Physiology will also be required to submit the following exam as a condition for admissions:
ANATOMY & PHYSIOLOGY
FORMAL
College Level Anatomy & Physiology with a grade of “C” or better.
INFORMAL
CURE Anatomy & Physiology Entrance Exam. This is a take home exam and any resources can be used to answer the questions including the internet, textbooks, medical professionals or other. A passing grade of 83% is required. Students have three chances to take this exam to qualify for acceptance into the CURE program. The average time to complete this exam is 50 hours.

REGISTRATION FEE
CURE does not charge an application fee. In lieu of the application fee, CURE is required to charge a registration fee. The registration fee for CURE is $100. This can be paid upon acceptance during the time of enrollment or before. The registration fee is non-refundable.

ENROLLMENT AGREEMENT
During the enrollment process, each accepted student is required to complete and Enrollment Agreement. The Enrollment Agreements allow the applicant to verify CURE’s offerings as well as learn about their responsibilities while attending CURE including the professional and ethical standards and other requirements.

CREDENTIALS VERIFICATION (If applicable)
Applicants for the short-term programs are required to provide verification of the sonographer/Allied Health credentials which includes registry cards from ARDMS or CCI, for example. Applicants for the long-term programs do not have to provide sonographer/Allied Health credentials unless applicable.

PHYSICAL REQUIREMENTS
See in very dim light.
Distinguish colors and subtle differences in shades of gray.
Discriminate subtle sounds.
Push heavy wheeled equipment.
Push patients on stretchers and in wheelchairs.
Assist in lifting, transferring and positioning patients.
Communicate in clear and concise English.
Read, understand and follow instructions accurately.
Be able to concentrate for a minimum of 6 hours.
Utilize a keyboard.
Have adequate wrist and hand strength necessary to operate & manipulate equipment.
Stand and walk for extended periods of time (several hours).
Employ body mechanics to bend, stretch, reach, stoop, kneel and twist.
The school reserves the right to reject applicants if the items listed above are not completed.
